Make an Appointment

Please call our office to schedule an appointment.  We try hard to accomodate work and school schedules, please plan ahead and book early to ensure a time that is convenient for you and your family.  We use an automated calling service to remind you of upcoming appointments or when it is getting close to time to schedule your next appointment.

What to Bring to your Appt:
Please show up to your appointments 10 minutes early to complete paperwork, bringing with a photo ID and a current insurance card or Medicaid stickers. You may also set up and log into your portal at your first visit from our office, where you can view documents, send messages to your provider, schedule appointments and pay your bill. 

Rescheduling Appointment or Late Appointments
If you can't make your appointment, please call to cancel or reschedule so we can open that time slot for other children waiting to be seen.  If you arrive more than 10 minutes late, please be aware that we may have to reschedule your appointment so we do not make the next patient late. We know that all of our patient’s time is valuable and want to maintain a timely schedule for our patients, please allow for travel and parking time to arrive to your appointment on time.

Insurance/Payment Information
As a courtesy, claims will be directly submitted to your insurance carriers. Check your insurance plan booklet for participating providers. It is your responsibility to contact your insurance company to find out if your deductible has been met for the calendar year, and what your co-pay is (percent you pay). Patients are responsible for deductibles and co-pays at the time of service.
